Do you accept insurance?

Renewed-Self accepts the following insurance in the following states (Utah/Arizona): Aetna, Cigna, Regence BlueCross BlueShield, BlueCross BlueShield of Arizona, BlueCross BlueShield of Massachusetts, Quest Behavioral Health, United Healthcare, Oxford, Oscar Health, and (Soon To Come, Select Health).

Renewed-self accepts Health Choice Medicaid, Medicaid CAP, Utah Fee for Service Medicaid, Southwest Behavioral Heath Medicaid. (And soon to come, Molina Healthcare Medicaid and University of Utah Medicaid.)

For those whose insurance is out of network and would like to work with Renewed-Self and upon request.  Renewed-Self can provide clients an invoice that clients can take to their insurance for possible reimbursement.

What payment forms do you accept?

Renewed-Self accepts cash, check, Venmo, Visa, Discover, Master Card, and HSA. For further payment questions or assistance, please email chase.brewer@renewed-self.com

When paying with a credit card, client agrees to pay for the additional credit card processing fee’s/taxes that are added to the session fee.

Renewed-Self retains the rights to change said prices at any time and will give a 30 day notice to clients if/when these changes will take effect.)

What is your cancellation policy?

Renewed-Self does not currently charge any late 24-hour rescheduling fees or cancellation fees if you are a paying out of pocket. Renewed-Self charges $90 dollars for each session. Here is how this will work, a $30 dollar non-refundable payment will be made to reserve a scheduled slot and the remaining $60 dollars will be required at the time of the appointment to complete the session fee of $90 dollars.  This design is to help clients schedule and keep their appointments.  If an appointment does need to be rescheduled, the old appointment time/day for that week will be forfeited and client can reschedule an appointment within a 3-5 day window where their $30 will follow, after 5  days, client will need to pay another $30 dollars to schedule and reserve a new time slot and then pay the remainder of the $60 during the appointment to complete their session fee of $90.

If you are using your insurance to pay for your sessions then, Client understands and agrees that a fee of $30 will be charged for any sessions scheduled where the client does not show up or where the client fails to cancel or reschedule at least 24 hrs before their scheduled appointment.

What if I need to reschedule my appointment?

If you are paying out of pocket and an appointment does need to be rescheduled, the old appointment time/day for that week will be forfeited and client can reschedule an appointment within a 3-5 day window where their $30 will follow, after 5  days, client will need to pay another $30 dollars to schedule and reserve a new time slot and then pay the remainder of the $60 during the appointment to complete their session fee of $90.

If you are using your insurance to pay for your sessions then Renewed-Self asks that you cancel or reschedule your appointment 24-hours before your original appointment.  Please be mindful that a $30 fee will be charge if it is after 24-hours.

Do you work with teens?
